To revoke a Code Signing ID without a Challenge Phrase, you must go through Manual Revocation. Please follow these steps:

MANUAL REVOCATION

Create a letter on company letterhead containing the following information:

Subject: REVOKE / REPLACEMENT

Common Name/Organization Name

Your request and the reason for the revocation:

- Lost/corrupt Private key

- Password not available

The type of Digital ID

Signature of the Technical or Organizational Contact of initial enrollment

Fax the letter to 1-650-961-8870. Within 24 hours of receipt of the fax, your former Code Signing Digital ID will be revoked.
